Publishing: added authorisation framework for channel types. Migrated YouTube channel type onto it, and added Twitter and SlideShare channel types. Facebook is also added, but there is an issue with the way it authorises apps, so it isn't wired in yet.

- cleaned the publishing context files out of the top-level application context
- removed the notion of "content root" from publishing channels
- added YouTube publishing channel with actions to publish and unpublish
- added dummy channel types to ease UI development
