Fixed ALF-4751 (When starting a workflow with no form configuration duplicated fields are seen), ALF-4859 (Lifecycle review and approve form is incorrect) & ALF-4649 (Node based forms render a form containing system properties when no configuration is present)
The set of default fields for each type of form processor has been updated, default node and type forms now don't return the 'sys' properties previously seen, a default task form no longer returns the identifier field and a default workflow form no longer returns the task level description, priority and due date fields. To avoid this we changed DispositionScheduleImpl. Now it stores the action that has different name and ID (when action is create it name equals id) in a separate map. And when getDispositionActionDefinition(String id) method is called it tries to retrieve the action from this map, if it hasn’t been found earlier.

Performance improvement for GET workflow-instances REST API.
Added "priority" and "dueDate" to WorkflowInstance object, which means a query for the workflow start task is no longer required to retrieve this information. Changed JbpmEngineUnitTest to extends AbstractTransactionalSpringContextTest. I hope this will result in Spring correctly cleaning up the JbpmFactoryLocator and stop other tests failing.

Changed test-context.xml so it no longer directly references the dev.properties file but instead overrides the repository-properties bean defined in dev-context.xml. This was done to fix an issue with the build not working.

Nick S: Created a JBPMEngineUnitTest which uses much less of the Spring context and is much faster to run.
Also created a OneToManyMap and a OneToManyBiMap to help mock up the NamespaceService.

The test requires a new database schema. The schema has the same name as the original schema but has _test after it, i.e. ${db.name}_test
