Adjusted AVMCrawlTestPP to be more stringent now that we have retryin
transactions.
History is noww correctly maintained for cases of deletes followed by renames
or creates.

AVMNodeDescriptor now can be asked what type of deleted node it is via isDeletedFile() and
isDeletedDirectory().

Lookup caching is working. It's fairly heavy handed in its invalidation
strategy, but seems to give a 10-20% performance boost.

Rejiggered ChildEntry and its mapping, for considerably better performance. Actually
it was kind of a Doh! moment.
